What is Coordinated Family Support (CFS)

Coordinated Family Support (CFS) program helps adults 18yrs and older with intellectual and developmental disabilities who live with their families by coordinating services, resources, and supports to meet their individual and family needs. It is flexible, person-centered, and aims to foster independence. CFS assist families in keeping their loved one at home.

CFS Services and Support

Our CFS program partners with families to identify their needs and provide tailored support across several key areas:

Assistance with coordinating and scheduling services.

Support in developing daily living and independent skills.

Support in accessing community resources and generic services, such as IHSS, SSI, CalFresh, Medi-Cal, and Social Security

Training for families and caregivers on supporting individuals’ needs.

Future planning, crisis planning and Advocacy.

Help arranging transportation and backup providers when needed.

Information and resources about diagnoses and available supports.

Respectful, culturally and linguistically appropriate service delivery tailored to family needs.
